FOR YOUR VISIT

Having the necessary information for your visit will allow us to accomplish more in a shorter period of time. 

LOCATION

The office is next to California Pacific Medical Center Davies Campus. It is the Medical Office Building across the driveway from the hospital main entrance. The neighborhood is called the Castro District. Parking is free on the street for 2 hours and there is a pay for parking garage next to the building.

PATIENT FORMS

The office will need many forms, including, but not limited to, a patient registration form, intake form with your medical and surgical history, office policies, and an arbitration agreement.

OFFICIAL DOCUMENTS

  • Official government ID

  • Insurance card

  • Hormone and mental health letters

  • Surgical and medical records, if necessary
