Consultations:

• Office: we will bill your insurance company for your consultation; patients will be responsible for their plan required co-pay, deductibles, and co-insurance.

• Phone/Video: we will verify your benefits to see if telecommunication is covered, if not, you will be considered self-pay

• Self-pay: $250

Pre-operative paperwork:

• Reasonable requests such work related letters and FMLA/disability paperwork are covered as part of your surgery.

Self-pay Surgery Fees:

• We do offer self-pay packages for patients. We are happy to review these fees with you following your consultation appointment.

Notarized Documents:

• Physician declaration letters or other documents requiring notarization will incur the cost of the services rendered by a notary public.
